Patient Outreach Specialist - Temporary (Remote within Mississippi)

Aledade is currently looking for full-time temporary support for a Patient Outreach Specialist in Mississippi. In this role, you will be conducting emergency department outreach calls, provide patient education about clinic appointment availability, and identify and communicate if the patient has any care coordination needs or triaging needs from the supported practice. All work will occur in partnership with the market and practice leadership team. As a Patient Outreach Specialist, you will be expected to work 40 hours per week for roughly 6-12 months.
This is a remote position & candidates must be located in Mississippi.The duties for this position are:
All prospective hires will be required to demonstrate that they have been fully vaccinated, including booster shots, against COVID-19 with a COVID-19 vaccine for which the U.S. Food and Drug Administration has issued a license or an Emergency Use Authorization prior to mutually-agreed upon start date at Aledade, unless they qualify for a medical or religious accommodation to this vaccination requirement. In certain limited circumstances, Aledade will accept documented proof of prior infection with COVID-19 in lieu of a booster shot.
