Position in this function is responsible for the coordination of outpatient services following an inpatient stay. The Inpatient Care Coordinator works directly with the member, provider(s), facilities, and other entities to ensure the most appropriate care is provided. The Inpatient Care Coordinator assists members from all contracted product lines. The Inpatient Care Coordinator meets with facility treatment teams and works with HPN utilization staff and Case Management staff to ensure member's needs are met upon discharge. This would include scheduling outpatient appointments, determining if member has current outpatient providers, ensuring member has access to transportation to attend appointments, works with member's family/support system to ensure positive outcome, and coordinates any additional social determinant of heath needs. The Inpatient Care Coordinator provides education, advocacy, communication, and resource management and promotes quality, self-management, and cost-effective interventions and outcomes.
