Job Title: Case Manager - ICMS/Housing Navigation Location: Downtown Los Angeles, CA (Skid Row) Dept: Housing Department Reports To: Program Manager Status: Hourly / Non-Exempt, Benefits: Medical, Dental, Vision, Retirement Planning (403b) EAP SUMMARY As part of a multi-disciplinary team, the Intensive Case Management Services (ICMS) Case Manager/Housing Navigator will engage with high acuity clients in the Skid Row Community and work with clients to attain and sustain permanent housing in partnership with the County Department of Health Services (DHS). The ICMS Case Manager/Housing Navigator will work with clients to assess housing stability, provide stabilization services, and coordinate multiple services to meet each client's specific needs related to housing, health, mental health, and income. WORK ENVIRONMENT 1. Combination of field and office environment 2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs. 3. On occasion walk or drive to different local sites 4. Will necessitate working in busy and loud environments 5. Will be exposed to elements like cold, heat, dust, noise and odor 6.
