We are searching for a Community Health Worker who, under the general direction of the Manager of Care Management or designee, will work in collaboration with a multidisciplinary care team to provide culturally appropriate health education, coaching, support, information and outreach in a variety of settings including community-based, primary care, ambulatory and acute care settings as needed. Identify high risk patients, associated health needs. Facilitate the coordination of community resources to meet the patient's plan of care. Assist patients to practice healthy behaviors and access necessary social and medical support services and programs. Conduct home and community visits.
