This role assists the Admissions Team in our Convalescent and Rehabilitation Center to ensure a smooth and efficient process for admissions, discharges and room changes. This position demonstrates a warm, welcoming and approachable demeanor with residents and their family members and collaborates frequently with other staff.

JOB DUTIES:

  • Greet new resident upon admission and assist with orienting resident to their room. (transferring, dressing, call button, remote/bed controls, etc). Take new resident photo.
  • Obtain vitals and record accurately.
  • Inventory and photograph resident's belongings.
  • Ensure rooms are set up prior to admission arrival and new resident has proper equipment available. Communicate with unit managers regarding specialized equipment and ensure items are in resident rooms.
  • Document in electronic medical record as required.
  • Assist with resident room moves when requested, ensure belongings are accounted for. Update medical records as appropriate.
  • Communicate with current resident regarding new roommates, obtain consent and document appropriately.
  • Provide assistance to residents discharging from facility, packing or loading belongings. Return equipment to appropriate departments.
  • Clean and disinfect equipment. Strip beds, collect dirty linen. Collect Sharp containers.
  • Conduct audits and maintain logs as instructed
  • Request, receive or send orders as appropriate.
  • Communicate with Benevolent Fund regarding donated items.
  • Assist as a floor NAC as needed.


REQUIREMENTS:

  • High school diploma/GED required.
  • Current Nursing Assistant Certified required.
  • Experience in long term care or rehabilitation facility required.
